Abstract

During the 35-year study period, 74 patients with non-traumatic spleen diseases underwent conventional total splenectomy for various underlying conditions. Three of these patients developed fulminant post-splenectomy sepsis. The prevalence was 0.7 patients per 100,000 adult inhabitants. The median age was 33 years, with a predominance of females (66.6%). Two hematological diseases were diagnosed (66.6%), one microspherocytosis and one immune thrombocytopenic purpura (ITP), and one non-hematological case, a splenic abscess (33.4%). All three patients developed bronchopneumonia, and the causative agent was Streptococcus pneumoniae. Our patients shared the following common features: loss of consciousness, septic shock, disseminated intravascular coagulation (DIC), acute kidney injury (AKI), and multiple organ failure. The patient with microspherocytosis died at 75 hours, the one with ITP at 97 hours, and the one with the splenic abscess at 76 hours. The morbidity rate is 4.0%, the mortality rate is 100.0%, and the death rate is 0.7 deaths per 100,000 adult inhabitants. A review of the topic is presented, highlighting its most important aspects
